NEW PATIENTS

New patients are always welcome.  If you are a new patient or have not visited us in the last three years, you will need to complete the New Patient forms in the forms section below.  Please complete and return these forms to our office prior to the day of your first appointment.  To help avoid delays or problems, bring the following information with you to your appointment:

  1. If not sent prior to your appointment, the completed Patient Registration Form, Medical Questionnaire, and Acknowledgement of Receipt of Notice of Privacy Practices.
  2. Your most current insurance card(s), and photo ID.
  3. Any required physician, insurance, or employer referral authorizations.

APPOINTMENTS

You may call (864)269-3333 to schedule an appointment for any of our offices.  If you need to reschedule or cancel any appointments, please provide us at least 24 hours notice.  Please make sure that you bring to your appointment a list of your current medications, insurance cards, photo ID, and any referral authorizations.

EMERGENCY CARE

Our doctors remain on call after regular office hours.  Please call (864) 269-3333 to reach the physician’s answering service.  A doctor will then return your call as soon as possible.

PAYMENTS

All co-payments, deductibles, and fees for non-covered services are due at the time of visit.  For your convenience, we accept cash, checks and all major credit cards.  

INSURANCE

We are contracted with most medical insurances and some vision plans. For plans in which we are not contracted, we can explain what effect this may have on payments due.  It is imperative that we review and obtain a copy of your medical insurance or vision plan card(s), even though your insurance may not have changed.
